Targeted Case Management is a service only provided by Critical Access Behavioral Health Agencies (better known as CABHAs).  As a CABHA, Bridges of Hope provides Targeted Case Management to children, adolescents, and adults who struggle with mental health or substance abuse symptoms.  Targeted Case Management is designed to assist with linking to resources or services needed, advocating, monitoring service delivery and symptom management, promote independence, and build support systems including natural supports.  Targeted Case Management may be a tool that can help when needing to be linked to housing, benefits, services or other resources.
