Partial Hospitalization is a intensive service that is available 5 days/week 4 hours each day to provide a structured illness management and recovery based treatment to individuals who are at extreme risk for hospitalization or re-hospitalization. This service is under the direction of our Medical Director/Psychiatrist but includes an array of professionals including Program Directors, nurses, therapists, qualified professionals, associate professionals, paraprofessionals and peer support specialists. Partial Hospitalization is provided to give the consumer everything he/she would receive in a Psychiatric Hospital including, medication management, medication administration, structured illness management classes, therapy (group and individual) and development of coping skills to de-escalate crisis and help the individual stabilize symptoms that are impeding his/her ability to live in the community. Our Hospital Liaison is a unique benefit that sets Bridges of Hope aside from other agencies in that she is available to assist individuals in hospitals with discharge planning and link directly to our Partial Hospital whether it be through outpatient commitment or discharge. This service offers a bridge from the hospital back to the community and can also be a resource prior to being hospitalized. Partial Hospital allows for the individual to receive hospital treatment in a licensed facility but return home during the evening. (Transitional Housing may be an option for individual who do not have a home to return to ). |
