This service is intended to assist Adults who struggle with severe mental health and substance abuse symptoms.  Bridges of Hope Community Support Team uses cognitive behavioral therapy as an evidenced based practice through a single organized unit of clinicians for each individual served.  The service is designed to help each individual served with developing coping skills for their symptoms.  The team works together to provide Crisis-Contingency Planning and Interventions, and incorporate Recovery Skills training.  CST works together with each individual to assist with payee support, housing stability, developing and maintaining relationships and natural supports, accessing other services needed (such as mental health, medical, dental), and assisting the individual with learning more independence needed to remain free from arrests or institutionalization.  Our team will provide wellness education and cognitive behavioral techniques to assist the person with improving his/her own ability to respond safely to his/her own symptoms.  Community Support Team is designed to assist adults with serious mental health symptoms who are at high risk for becoming unable to remain living at home or in the community without this level of assistance.  Bridges of Hope Community Support Team will work together with the individual in need to provide motivational interviewing, treatment team meetings and linking with natural supports in the community.  Each person is unique in the areas of need for support of Community Support Team but the time spent with the team will be mostly face to face and in the community or home setting.
